This is one of 5 promotional singles (PRO 514, Record I to Record V), issued in a press pack at an end-of-year 1971 dealership party in Houston, Texas.
Each picture sleeve (different colors) had a 'head' shot of the artist & the first characters of their names; i.e. "Beef" = Beefheart, "Gar" = Garcia, "Coo" = Cooder, etc.
These singles were later distributed in early 1972, either in remaining promo sleeves, or generic sleeves, or loose.
